• ​Apply digital technologies to facilitate patients remaining at or returning home after acute or post-acute inpatient hospitalization

 

• Develop virtual care, remote monitoring, and telehealth solutions to connect assisted living/skilled nursing patients with clinicians to reduce anxiety and cost of unnecessary emergency room visits/readmissions

 

• Enhance use of digital technologies to improve the flow of patient, caregiver, and clinician communication allowing providers to drive peace of mind and differentiated service offerings (subject to HIPAA)

• Appraise and determine appropriate use of remote-monitoring and virtual care technologies to connect Medicaid/Medicare and Dual Eligible Beneficiaries with community providers to reduce costs and improve quality of care
